Policy: No Cancellation: Enrollment in the Nutritional IV Therapy Program is final and cannot be canceled. Transfer Option: Enrollment may be transferred under approved circumstances. Requests must be made in writing and approved by Sei Favolosa, LLC. The new participant must meet program eligibility criteria. A transfer fee may apply. Transfer Procedure: Original participants must submit a written transfer request to Sei Favolosa, LLC, including details of the new participant. Sei Favolosa, LLC will review and notify both parties of the decision. If approved, the new participant completes necessary paperwork and meets with a healthcare provider. Continuation: Transferred participants continue remaining IV therapy and injections from the original participant's point.
